Why a diagnosis is the beginning, not the end
There is a common misconception that once bone density starts declining, the right response is to slow down. Move carefully. Avoid anything that might cause a fall.
The evidence says the opposite.
Bone is living tissue. It responds to mechanical load. When you apply the right kind of stress through exercise — specifically progressive resistance work and controlled impact activity — bone-forming cells are stimulated and remodelling is triggered. The result, supported by decades of research, is measurable improvement in bone density and a meaningful reduction in fracture risk.
The key word is right. Not all exercise produces the mechanical stimulus bone needs. Low-intensity walking, gentle yoga, and recreational swimming are valuable for general health, but they do not load bone sufficiently to drive density improvements. Physiotherapy-led programs are designed specifically to close that gap.
What Does Bone Health Physiotherapy Involve?
At BeFit, bone health physiotherapy is not a generic class or a one-size programme. It starts with a thorough assessment and builds from there.
Your physiotherapist will review your bone density results (DEXA scan data if available), your fracture history, your current medications, and any other conditions that affect how your program should be designed. From there, a structured exercise program is built around your specific starting point.
The core components of a well-designed bone health program include:
Progressive resistance training Lifting progressively heavier loads through compound movements — such as deadlifts, squats, and pressing patterns — is one of the most effective ways to stimulate bone-forming activity at the hip and spine, the two sites most vulnerable in osteoporosis.
Impact and balance training Controlled jumping and hopping activities, combined with balance and coordination work, address two things at once: bone stimulus and falls risk reduction. Falls are the primary mechanism behind osteoporotic fractures, so improving balance is as important as improving bone density.
Posture and spinal loading Vertebral fractures are common in osteoporosis and often occur without a specific traumatic event. Your program will include targeted work to support thoracic posture and reduce compression loading through the spine.
Progressive overload Like any training program, the exercises are progressively loaded over time as your strength and capacity improve. Your physiotherapist monitors and adjusts the program at each session.
Bone Health Physiotherapy and DEXA Scans
A DEXA bone density scan at our Edgecliff clinic] gives your physiotherapist the precise data needed to programme your exercise safely — specifically your T-score at the hip and lumbar spine, which determines your fracture risk profile.
If you have a current DEXA result from your GP or specialist, bring it to your first appointment. That is all you need to get started.
If you do not have a current result, your GP can refer you for a scan. BeFit also offers DEXA scanning at the Edgecliff clinic, which takes approximately 10 to 15 minutes and may be bulk-billed with a Medicare-eligible referral.
You do not need a DEXA result to book an initial assessment. Your physiotherapist will discuss the most appropriate next steps with you during the consultation.
Your First Bone Health Physiotherapy Appointment
Your initial assessment at BeFit is a structured, one-on-one session. Depending on your history and where you are starting from, it typically covers:
- Review of your bone density results, fracture history, and current medications
- Functional movement screening — an assessment of your strength, balance, posture, and movement capacity
- Falls risk assessment, including balance and coordination testing
- Discussion of your goals and what you want to be able to do
- Initial program design, with a clear explanation of what the program involves and why
The assessment takes approximately 60 minutes. No referral is required to book, though bringing any recent DEXA results or GP notes is helpful.
Why Bone Health Physiotherapy Focuses on Exercise
It is worth addressing the instinct many people have after receiving a diagnosis: to protect the body by doing less.
Current guidelines from Osteoporosis Australia and the Royal Australian College of General Practitioners recommend structured, supervised exercise as a core component of osteoporosis management — not an optional add-on.
The reasoning is straightforward. Inactivity weakens both bone and the surrounding musculature. The less you move, the higher your falls risk and the faster bone density tends to decline. Supervised, correctly loaded exercise is one of the most protective interventions available.
This does not mean ignoring pain or overriding fracture history. A well-designed physiotherapy program accounts for your specific risk profile — loading appropriately and progressing safely, with your physiotherapist monitoring at every stage.
If you are taking bone-targeted medication such as bisphosphonates or denosumab, exercise complements pharmacological management rather than replacing it. Many people manage bone health effectively through a combination of both.
Bone Health Physiotherapy in Sydney’s Eastern Suburbs
BeFit operates two clinics in Sydney’s Eastern Suburbs.
Edgecliff is BeFit’s primary clinic and offers the full range of bone health services, including DEXA bone density scanning, physiotherapy-supervised exercise programs, and for eligible patients, access to structured bone-loading programs with ONERO-accredited practitioners.
Coogee offers injury rehabilitation and general physiotherapy, including bone health assessment and supervised exercise programming for people managing osteoporosis or osteopenia.
Both clinics are staffed by experienced physiotherapists who assess and programme bone health individually. Your first step is an initial assessment at whichever clinic is more convenient for you.
Common questions about bone health physiotherapy in the Eastern Suburbs
How long does a bone health program run? Bone health is a long-term process. Most people benefit from ongoing, supervised exercise for 12 months or more. Clinical benefit accumulates with consistent attendance and progressive loading. Your physiotherapist will review your progress regularly and adjust the program as your strength and capacity improve.
Can I start a program if I have already had a fracture? This depends on the nature of the fracture, when it occurred, and your current bone density results. A thorough initial assessment will determine the most appropriate starting point for you. Many people who have experienced fractures do progress to full loading programs with appropriate preparation and monitoring.
Is physiotherapy for osteoporosis covered by private health insurance? Physiotherapy consultations at BeFit are claimable through most private health funds. Coverage varies by fund and level of cover — check your policy or contact your fund directly. DEXA scans may be bulk-billed through Medicare with a GP referral.
Do I need a referral to book? No referral is required to book an initial assessment at either BeFit clinic. If your GP has referred you for bone health management, bring the referral and any associated results to your appointment.
Where are the BeFit clinics? BeFit Edgecliff is accessible to patients from Double Bay, Woollahra, Darling Point, Rose Bay, Paddington, Potts Point, Surry Hills, and Rushcutters Bay. BeFit Coogee serves patients from Randwick, Maroubra, Bronte, Clovelly, Kingsford, Bondi, and surrounding suburbs.
What should I bring to my first appointment? Any recent DEXA scan results or specialist letters. Comfortable clothing you can move in — the assessment involves movement screening and light loading.
Staying active in the Eastern Suburbs with osteoporosis
Sydney’s Eastern Suburbs is home to a large, active population of adults over 50 — people who swim at Coogee, walk the coastal tracks, play tennis, and have no interest in slowing down. A bone density diagnosis does not have to change those goals.
With a structured physiotherapy program behind you, many people continue doing the activities they love while actively working to improve their bone health. The aim is not to restrict what you do — it is to give you the foundation to keep doing it safely and for longer.
At BeFit, we work with people who are newly diagnosed and unsure what to do next, as well as people who have been managing osteoporosis for years and want a more structured, evidence-based approach. Both are valid starting points. The work always begins with an honest assessment of where you are.
Ready to take the next step?
If you have been diagnosed with osteoporosis or osteopenia — or you simply want to understand your bone health better — an assessment with a BeFit physiotherapist is the clearest next step. We will review your history, assess your movement and strength, explain your options, and design a program suited to where you are right now.
Both the Edgecliff and Coogee clinics are available for initial assessments.
